  Registration Form & Personal Records 

  Original form of NJA. Click here to download the form. Hand written by   applicant.

  Official photographs

  4cm×3cm. 8 prints. Taken within the last three months with your   name written on the back. Pictures printed from computers are not   acceptable.

  Original diploma of the last academic career.

  No copies permitted. Will be sent back to the applicant.   

  Mark sheet of the last academic career.

  No copies permitted.

  Certificate of studying Japanese.

  More than 150 hours at any Japanese school.

  Copy of passport

  If you have one or previously been in Japan.

  The certificate of relationship between the applicant and the sponsor.

  The original family registers or birth certificates to show the relationship   between the applicant and the sponsor by the government and other official   department.

  Documents    of the   person   bearing   study and     living   expenses
  (sponsor)

  Guarantee for Bearing Study and Living Expenses

  Original form. Hand written by sponsor.  

  Bank balance certificate.

  More than the equivalent of 1,500,000 Japanese Yen.

  Copy of bank note

  If you have one.

  Certificate of employment or business license certificate.

  Certificate to show the occupation of the sponsor. If the sponsor is an   independent businessman, or a self-employed person, business license   certificate is required.

  Income/Tax Certificate

  Document to show sponsor's income of the last three years.

 Special Notes:
(1) If you apply through a proxy in Japan, we will ask the proxy to come to NJA directly.
(2) Except for the original diploma, no paperwork will be returned to the applicant. If there are any documents that must be returned, please tell us when you apply.
(3) On the all copies, you have to state the date of copy, the name who copied, and the relation between applicant and the person who copied.
(4) All paperwork must be officially translated into Japanese by word processor (handwritten translation not accepted).
(5) Providing false statements will result in cancellation of admission and the student will be ordered to leave Japan.
(6)When you copy documents and submit it, please make it A4 size.( please write the day ,that you copied and your name in a blank.)
(7)Please submit the certificate with the address and phone number of a publisher which should be written as much as possible.
(8)When you submit a certificate translated into English, please submit the original.
